I usually start by deleting all the default materials that came with the model, but there is always one material that is impossible to delete. You got me. All I know is that I can't replicate your issue. I can clear all the Materials out, save, close and reopen the rfa - or change it to an rft and use it as a template - and still there are no Materials in it.
If it isn't an add-on, try this: close Revit, rename the Revit.ini file or move it to another folder and restart Revit. Then open the version of the file that you know for a fact doesn't contain any materials.
BTW: to establish it isn't an add-on causing, disable Add-ons.
How does that solve the problem, which was your inability to delete all materials from the Template? You should still be able to delete the Material if it's assigned to an Object Style. I did.
