I have a cast in place concrete stair on top of a modelled in place stair component. I would like to extend the railing from the revit stair to component stair, but it does not follow the slope. I did not break the railing path until hitting the bottom extension (you can see the break in the railing in the view). I also cant pick multiple hosts or can I?
Any help would be appreciated.

Assuming that your model-in-place stairs are at the same slope as your CIP stair, then all you need to do is extend the railing sketch without breaking it. No need to add an additional stair. The railing will host to the upper portion of the stair. See attached images.

I would have expected this result, but unfortunately have not been able to just extend the stair. It extends flat. Slopes are the same. I used the property riser/tread dimensions in my model-in-place. Still flat. I did your first option though and for SD, this works. I'll redo the whole thing and see if it works then.

I think the problem is there is already a split landing. When I edit railing and "extend" the line of the bottom railing, it goes flat regardless.
