Is it possible to add a line in the 'level head' to add a structural height similar to the 2nd image below?

(right now, I have it as a spot elevation)

FWIW: Both Elevations are relative to the same Elevation Base (Project Base Point or Survey Point).  If you want the Elevations to be relative to different Elevation Bases, then Spot Elevation is a good/common workaround.

Since I can't get the correct elevation to show, can the spot elevation to show up as " 100.00' "

either it shows as 0' or 390.150'.   the client would like to see '100.0'  instead of 0' (relative).

can this be done?

100' from what?  What is the 100 ft. relative to/measured from?

Well, the "100'" has got to have some meaning.  What does it mean?

You could create a Project Parameter assigned to Levels from a Length Data Type Shared Parameter that is also used in the Level Marker Family. Get it? It wouldn't be "intelligent" though. You would just enter some random value into the Project Parameter Field under Level Instance Properties.
