Note Block Symbols

Trying to create symbols, using generic annotation family, to use in a Note Block schedule. Anyone know where I can find a tutorial on how to create one? Basically I want a hex symbol with a tag in the middle to be visible with the description of the tag number to be visible in the Note Block Schedule only. Any help would be appreciated.

I forgot to make the note content invisible.

Thank you. I was close just didn't unselect visible in the family. I was trying to do it in the project file.
