I am trying to mask the background of everything inside the text box shown below. Can anyone explain how to do this please?
You would just add a Masking region to the family. only catch is it will not adjust automatically with text in the family. The mask will be static regardless of the length of the text line or how many lines you may, or may not, have.
You can add some Type parameters to the family and lock the masking region to it, but their Type based, Not Instance. Instance parameters won't be editable. (see attached).

The background that is part of a text type only appears behind the text itself, even if you have a border turned on.
This is a long standing request to have the option to fill the entire boundary of a text box.

For now you will just need to create a masking region and place it in the same spot as the text. You may also want to group it so they stay in the same position relative to one another.
Or as @hmunsell you could make this as a generic annotation family with a filled region and a label using a multiline text parameter.
