This is something I've done a workaround on for years. Finally thought I'd get round to finding a 'proper' way to solve the problem (if one exists).
I have structural elements (eg dimensioned timber joists) which I place in a model. When I cut a section throught the model and draw insulation batting lines they are placed over the cut joists.
I have added a masking region in the structural family but this only masks other model elements not detail items.
My current workaround is to draw masking regions in the section view over the end of each structural element which is time consuming and has to be one on every section and call-out and edited if there is a change to the size or position of structural elements.

That was my second choice of workaround but its messy with insulation lines not starting and stopping in the correct position and the same problem of redoing the work if there is a change to the position or size of the structural element.

Absolutely, me too. My brain thinks in real world site mode so that's how I model/draw.
As you say, it's not always possible.
If a revit floor could have joists and insulation in the same layer (as in the real world) that would be amazing!
This masking/detailing issue is only a minor bugbear so I won't go on (much longer). The problem with drawing insulation between joists is the lines not ending in the correct place - either overlapping the joist or being cut short (see image). If you know a way around this that would be great.
Maybe draw a ref plane down the centreline of the joists and then you can align the insulation segments to the ref plane.
