For the past few CAD files my civil engineer has sent me, I have not had any of the "Manual" options in the Positioning drop-down box. Why is this option sometimes available and sometimes not? I'd like to be able to manually place the files.

Are you going to Insert=>Manage Links=>Add.  No manual when you do it that way.

Just click on Link CAD on the Insert Tab.
