What do you mean by wall extrusion? Did you model in place a wall instead of using the wall tool? If that is the case, be sure to set the model as a wall category in the properties (yellow folder in tabs)
If you are drawing your wall wall, by Component, Model in place, extrusion or any other forms, be sure to choose Wall category in the family category and parameters window. If you draw it as the wall then Revit will know it as the wall and the door will sit on it like the system family walls no need for opening. You should consider that if you do not draw it in the wall category you wont be able to put door on it.

Good suggestion! However, if the model in-place wall is of an irregular shape, the door may not be able to cut it still. In such case, he needs to manually add an opening.
