I am wanting to create a custom door/window tag that include the width and height of the unit, but with the format that shows the attached picture.

I think they only way would be to use one of the other Type properties of the door and to place the values there and to make a new tag family that looks at that Type property. One problem I do see is finding a character in Character Map that is a super script that you would have to then paste in the Type property.
Is this specific look mandated by your client? If not I would then change to a more normal door tag and have the door schedule have the properties of the door. Sure having the size right on the plan is nice, but then you take away from the power of schedules and smart tags.

Revit is not AutoCAD and you will find it might be difficult to replicate everything from AutoCAD to Revit. AutoCAD is a DRAFTING program. Revit is a INFORMATION DATABASE program.
