When i use flip/mirror (double arrow icon). My handrail loses it's graphic properties. It works flawless in shaded Visual Style, but in Realistic it starts to freak out. Why does this happen? I would like to present it in shaded but then my mesh pattarn won't be shown. Is there any to change so the pattern can be showned in visual style for shaded?

I can reproduce it in your Project if that's what you are wondering. I can reproduce in a lot of Projects. It's quirky behavior but not that big a deal to me because I can quickly and easily fix it by just toggling the Visual Style. I guess you could contact Autodesk about it. My guess is that they would just give you the ol' "This incident is being investigated for a possible cause and resolution" answer.

In answer to one aspect of your question:
There are a lot of times when Revit doesn't work well even when you are following all of Revit's rules. In this case you're sidestepping those rules to get Revit to function the way you want it to (and how it should work to begin with in my opinion). IE: you are using Handrails as Top Rails so you can have manually adjustable balusters (as handrail supports).
A good workaround for a shortcoming in the system, but all workarounds (such as this one) can lead to confusion on the part of Revit's programming and thereby create unexpected results.
