You can create a TYPE that does that, or another Family. But NO, you can't do in "on the fly" per placement instance in the Project.

If you choose to have 2 TYPES within the one Family, you can use Visibility Parameters and a simple formula to control which side is Visible for Types 1 and 2.

....just to clarify, you need to Create another Level Type in the Project that uses the different Head -- which you need to create.  Then you can use one of those two Level Types in the Project -- but not both for the same instance/Level Type.

I'm new to revit and I would like to know what would be the formula to control de visibility if you don't mind sharing it.

Select the element and look under its properties. See "Visibility" checkbox? Well, that Visibility YES/NO Parameter can be associated to a Family Types YES/NO Parameter.  Does that help?

I'm familiar with the per type, what i don't follow is the or may be i'm misunderstanding something.

Can you have only one spot elevation family that does the flip automatically? or you must have two?

Regards.
