I am in the process of cleaning up a toilet family we want to use for our office's BIM Standard Family, but seem to be running in to some trouble with how this specific family looks with the 'Course' detail level setting on.
It is a very simple toilet family with a bowl, seat, and flush lever that we based off a Kohler model. Below are screenshots of what the toilet looks like at a fine detail level:
medium detail level:
and course detail level:
When it is set to course, the geometry becomes just a rectangle with no resemblance to a toilet, and we don't want that. Is there any way to grab the model geometry of the bowl portion of the family to make it so that it appears when it is on the course detail level?

Edit the family, delete the box or make it invisible, select the actual toilet, edit Visibility/Graphics and check any boxes you want it to appear on.
Doing anything to the box ends up making the whole toilet disappear, no matter the detail level.
Maybe the box and the toilet is with another family nested in the parent family. Can you share the file?
The toilet bowl and toilet seat are nested families. You need to edit them and do the same as I said in the previous reply. Revised file attached.
