Operating System: Win 10 / 64 bit
When working with detail items the blue circle of death appears for every single action and lingers for 2-5s making working in Revit almost impossible. All add-ins have been disabled and removed from Programdata/Autodesk and other addin folders. In the options panel I have also reduced the "Worksharing Update Frequency" to the smallest option. I am at a loss what to do at this point.

I understand you are seeing a hang-up in Revit (spinning circle) after each action when working with detail items in Revit 2021.1.2 (and you've already tested disabling add-ins and setting worksharing update frequency to manual).
Another thing to check is if this behavior is specific to the model. You could do this by creating a new model using a default template.
If the model / detail items are stored in the cloud or on a network server, it would be good to test copying these locally to eliminate network/cloud issues as playing a part in this behavior.
If the info above does not lead to an answer on what is causing the hang-ups, could you attach a journal file recorded when this is happening?

One thing I'm noticing in the journal are entries like the following:
Line 224899: ' 1.329448!!!!!! 5:!!!BIG_GAP BorrowElements::borrowFromWebServices
This is indicating that it is taking over a second to borrow an element for editing.
If this is happening each time you are trying to modify detail items this may explain the delay you are seeing.
To confirm, try manually taking ownership of elements before modify them. For info on this process see the help topic: Borrow Elements
If the blue circle shows up when clicking to make the element editable, then that would confirm that it is the borrowing of the element which is causing the hang up. If this is confirmed, I would suggest taking a look at your Internet connection (both bandwidth and ping times) to see if it meets the minimum recommendations for cloud worksharing. If you are working on a wireless connection, try switching to a wired connection (if available).
