Just wanted to ask if it is possible to add column/s on sheet issues/revision in revit?

Sure it is. Just add parameter fields to the "schedule".
...what are you wanting to add?
9 columns to be exact. Rev., Emission Type(E.T.), Description, Designer, Drafter, Ver., Apr., Aut. and Date.

Is Emission Type value per project or per sheet? If per project then you can add a Project information parameter to the schedule. Plus, enter Designer/Drafter/Ver into a single field such as Issued By. Apr. = Issued to. Not sure what Aut. is but you can use Project information > Author. There you go.
The Emission Type, Aut. (Autorized By) values is per project but the Rev., date, ... are per sheet values.

Then what shown above should work perfectly fine..
Create a new field is unavailable. Edit(or delete) an existing one is greyed out. What do I need to do?

Seems redundant. Don't you have Checked By as Sheet's parameter already?
First of all, my initial query was that I am unable to add new parameters to Revit's in-built Revision Schedule. Is there a way to do this? I created a shared (type) parameter 'Checked by' in the sheet but it does not appear as one of the available parameters in the revision schedule.

I know what you initial query is and I know user's parameters cannot be added to revision schedules for the time being. My question for you was why do you need to add [Checked By] to your revision schedule when the built-in [Checked By] parameter already exists and can be shown on the sheet's titleblock?

Now I understand your previous query. So, the 'Checked by' parameter from sheet is used as Engineering check in our office and it exists on our title block. The checked by on the revision schedule is for drafting check and is checked by different person each revision.
What I have done for time being is to create a 'calculated' field named 'Chkr' and equalled it to existing 'isued by' field.

I thought you said you had no fields available. If [Issued By] is not being used then why don't you just use it and rename the Schedule Header to Chkr instead of adding a Calculated parameter, which will show the exact same value?
But is it not stupid from auto desk part in not providing to add fields for revision column , i ve drawn by , designed by , apprd by col in addition to issue by and issued to columns. how to insert all this , autodesk support to answer
Edit your titleblock family, edit the revision schedule, add fields to it.
but in revision schedule there is no provision to add extra field .. the filed should be sheet wise parameter not project wise.
can you please show how to add extra fields in revision schedule im using revit 2022
