Let me know if you find a solution to the custom size definition. I am struggling to solve this as well...
Hopefully we can find a way to define a custom size on the pdf print settings...

I guess the best solution would be to find an external driver for pdf printing that is opensource (Revit API only allows to take/read the print definitions that are already created)
