Extrude > Inset isn't working the way I want it to

In this video above, I can't seem to replicate what he does at 2:15 where he insets some faces by extruding with offset. I also need to point of that when he insets in the video, one object is selected and the other isn't (dark blue and light blue) I want to ask how he does this as it's a new mesh (one object) created using Mesh > Booleans > Union. When I do it, I get a rim around the cylindrical object like so:

you can´t replicate it because he´s not explaining it very well or lets say he maybe was to lazy to redo the first 2 minutes. he makes it look like he did the booleans before the inset but that´s not true.
you can see two things 1. there is an edit in the vid just before he´s selecting the faces to extrude and 2. the smaller cylinder has changed in size, most likely because he noticed there is not enough space for the transition to work nicely.

so basically you create your cylinders as seen in the tutorial, place the cuts like he did THEN do the inset/ offset extrude and then finally you can do the boolean union and all the following steps.
