Interactive Grooming Fly Away Hairs

I keep running into this issue where when using Maya's interacting grooming I get hairs that do not attach or fly away from my model. This issue seems random from scene to scene. See picture. Here in the picture I move my character over so that you can clearly see the hairs that are left there floating. What causes this and how do I fix it. Bear in mind I am using Maya Interactive Grooming. 

 

Is this a bug? I am using Maya 2024.

We need more info about your scene. How does the groom mesh look? How does your density map look like. Is the character riged?

Thanks for the reply. I am posting screen grabs that I hope will make things clearer. I am not using painted density maps or any painted maps. I just used the default of one for density on the groom mesh. Yes the character is fully rigged. Including full facial rig. The groom mesh is weighted to the face. I have also include a shot of the groom mesh.
