Vertices sticking in mid air.

Almost finished my rig but have a problem. The skinning on every joint seems good other than the arm. I have some vertices that have no influence (I think). I have painted at 100% but still they stay there.
I have tried the hammer tool but it says "// Error: line 0: Unable to perform smoothing on selected components: the topology has changed between the skin and the output."
I need this rig finished by tomorrow and I'm stressing.
Can anyone help?
