Pose space deformer bugged?

Hi wanted to ask about the pose space deformation tool. Is it somehow bugged?
After adding/enabling a pose it does that deformation for no reason. There are no corrective shapes done.
Followed this tutorial to start learning the tool. Deleting and mirroring does also not work
