Joints moving out of the mesh

Hi hope someone can help. I create a rigg and bind it to the mesh but when I move the joints they come out of the mesh. If I use IK handles it's the same Problem. I don't now what I can do to fix this.
Also if I want to animate a Leg and I push it up with the IK handle, the hip joint goes crazy and the leg turn around very fast.
Another Problem is, if I had IKs at the fingers and move the hand, the IKs of the fingers stay in place. I tried to parrent all the controller of the fingers to one controller but thats not working.
