Correct way of parenting elbow and hand controls to shoulder controls?

I am wondering how to get the elbow rotation controls, elbow pole vector, and hand controls to move with the shoulder control rotations. As of now, they currently stay in place when I rotate the shoulder while the mesh itself moves away from the two controls as seen (first picture).

 

I have tried parenting the shoulder to the elbow and the elbow to the hand but the elbows don't follow correctly all the time (seen in the second picture). The same thing happens with the pole vector, knee and foot controls for the legs (last picture).
