Button definition handler

Hello,
Whenever I try to attach an Icon to a button, the function fails. If I run
the SimpleAddIn sample, this to will fail, both situations fail unless I
remove the optional parts of the CreateButtonDefinitionHandler function.

Have I missed something?

There maybe problems using icons whilst in the debug mode, however all seems
to sort itself out when you compile and install the project.

Have a look for "Creating command button icons in addin" posted on the
16/07/2003, last posted on the same day aswell.

In debug mode the icons will not show. They are replaced by text buttons.
