Maintaining restraints issue

Why would a file revert to an unconstrained condition after constraining all objects.  Noticed this action after constraining parts and a few days later, or a different user goes in, the parts are no longer constrained.

Possible solutions or suggestions will be greatly appreciated.  Would a sample file help?

Have geometries changed in the constrained parts? Those could be broken if the faces/objects changed.

Could possibly be in a Positional Representation that has all the constraints suppressed. But that seems pretty unlikely.

A sample would certainly help.  And instructions for how to replicate the problem?

Could it be that constraints were added while not in "Factory Scope" for model states?

Hi! The behavior does not sound right to me. Please share the files that exhibit the unconstrained behaviors. I would like to understand it better.
