Sounds like you have the create new file dialog off screen.
When it appears its hung
Do the following steps
ALT+SPACEBAR
M key
Left or Right mouse key
Move mouse..
Is there a dialog trapped to your cursor?

Hi! Aside from out of focus command dialog like Mark mentioned earlier, another possibility is outdated graphics driver. Please make sure all critical Windows updates are installed and graphics driver is updated.
