Hello,
I have Plant 3D 2016 64-bit (7.09.4.04) installed
(no extensions or service packs). The Desktop App
is prompting me to install Extension 1.
It fails with the following:
This update could not be installed because it is not
designed for the version of Autodesk AutoCAD
Plant 3D 2016 Core that is installed on your computer.
One prior post on this issue suggested removing
extensions and service packs, which does not
apply for me. Any help would be appreciated.

Are you 100% sure you don't have a service pack 2 installed for Plant 3D or for vanilla AutoCAD?

Launch Plant 3D, do help/about and show the about dialog here. Make sure to remove any serial # from the image.
Usually that message indicates you have something newer already in place.

Please review the Read Me link, the extension is a subscription-only product. If you are a subscription customer, then try using the Autodesk Account to apply the extension you can locate it on the left navigation bar "Product Enhancements".
