Tool change crash from code

First time posting so bare with me please.

I am running fusion 360 with seimens 808D  post. I randomly have where the tool change goes into grab the holder and does not release pressure on the retention knob, thus it rips the holder from the tool changer rotates the magazine and knocks the other tools out.  This only happens with a random tool/holder locations. It can be loading fine, then the next program it has an issue with only tool in the program.

So my original thought was that something was wrong with the machine *which still could be a possibility* but the only way I have gotten this to be fixed is by completely redrawing the part in fusion and reprogramming it.

Is there something I am missing with the code? When I look through it, it looks like any normal tool change line.

So this morning I messed around with my setup. I programmed the two operations with different tools (will run the tools I actually need) and tool change went fine. No issues. Am I missing something here with programming my tools in the tool library? What can cause this? So in all, the post did not like those two tool callous?

That does not sound like a Fusion issue, but a tool knockout cylinder issue. Can you share the exact code it was running when it did this, contrasted with code that doesn't do it?
