Setup Sheet has Wrong Tool for Operation

Lately I have noticed that my setup sheets do not always include all tools required. If I look at the detailed view for the setup sheet, you can see that the wrong tool is displayed for a given operation.

The program posts correctly with the correct tool (T11 in this case)

Please attach your model you are seeing this issue in. It is a little hard to diagnosis you issue from what you have stated only.

Here is the file, however I just tried to replicate the issue from the exported file and was not able to, so I'm not sure this is going to be much help. Is there a more robust way for me to export the file?
