Issues retrieving Start-Up license for colleague

Hello,

 

We had applied for (and have been approved for) the startup license of fusion 360. The welcome message includes a link to a google form that should allow me to register more users under that license. However, despite resubmitting this form several times over the past few months, the account of my colleague has not been activated as of now. Maybe i have missed a step, however there were no further instructions included in the welcome mail. I would request some assistance with this.
