Form Tool Box has extra surfaces

Experiencing an issue where if I make a box with the form tool it makes these extra surfaces come out from the edges. Attached an image of the surfaces, an image of the box in the form tool without the surfaces. Also attached the file I'm using. 

The surfaces show up in render and when exported.

strange, I don't see that with this design.

but, this seems like it might be a graphics glitch.  Can you try changing the display detail control:

choose "fixed", and try just switching between, low, med, and high, and see if one of those makes this go away?
