Hello, and welcome to the forum.  I believe the form in your image gives you the option of selecting "No Company" as a response to the first question for those who are responding as individual users.  Is that what you are asking?

Well it did not have the option to select "No company". I really needed to update a part for my car because it did not fit, so I wrote something like "no company" in the form.

Probably not the right solution?
