How do i create hole patterns all the way around an organic shaped model based on a body part scan?

I am trying to create holes for breathability on a body that is built around the outline of a scan of a leg so is a bit of an awkward shape, when i use the emboss tool the pattern wraps around the body in a weird way.

this is what i want to achieve, (not from fusion, and doesnt have to be voronoi)this is whats happening when i test using the emboss tool

Any help that you can provide will be greatly appreciated.
