Work offset problem

Just posted the attached file and went to run it...for some reason the the tools are all starting appoximately 1.30 in  x+.

Luckily I was hoving over feed hold when the face mill did a  z- rapid in the middle of the part.  I double checked my offset on the machine it's good so I'm feeling stumped.  I also posted a picture to show where the face mill thought was the end of the facing pass Any ideas?

Looks to me like a Setup issue, the X and Y axis seem to have been transposed somehow, have a look at your G Code and check the actual numbers, first image is your original setup, the second one is with the setup swapped.
