Trouble setting up Amana chamfer bit

For some reason this bit isn't in Amana's master library and I'm having trouble setting it up manually. There is link to tool, picture and what I've tried in fusion.

To cut like an endmill and put a chamfer on the top of a part, I'd recommend creating it as a form tool, not a chamfer tool.

Thanks, Seth. I'm not seeing a form tool as an option anywhere. I click the + to add a tool and it lists the options. Can you please give some more details? thanks

@rhetteshleman You have to sketch the form tool. Its not an option on the new tool category.
