How did I achieve this face mill cutter geometry?

In the attached screenshot, I have the cutter page from a face mill I created in my tool library. It has a 45-degree tapered edge, but it's also got an outside diameter that limits the taper. This emulates the octagonal inserts in my face mill.

 

Unfortunately, I don't remember how I did this! Can anyone help me?

Did you define a custom shaft for the tool?

from the Shaft tab, but doubt its necessary with a face mill and its intended uses.
