Help Debugging - Cannot Create Items

As of today, I appear to no longer be able to create items.  I can select the "+" icon to create a new record, and fill out the form normally, but when I hit save, I just getting the "working" spinning circle icon indefinitely:

It does not appear to time out (I have let it run for 15 minutes before refreshing the page) and no errors are reported.  I have reviewed the itemnumbergeneration script, and everything appears as it should. I have not made any changes lately, so I am not sure where to look next.  Please can someone share some advice on how to debug this scenario?

Based on your initial assesstment, this seems like a potentail bug.
