Change Order closed but still in working state

Hi,

I might have encountered a bug or just made a mistake somewhere. I created new change orders for both a part, and the accompanying drawing a while back. Both saved as milestones and then fast track approved them. In the process work field they are both approved and closed with a revision letter D. However, when opening them in Fusion they are in a working state without any revision D available. 

I cannot create a new CO as it refers to the existing ones that are already closed so I am now stuck and am hoping someone here might have a solution to what I can do to resolve the issue.

Could you please share a picture of the Change Order workflow used and one of the tab "Managed Item" of this CO to check few things?
