Cannot access attachment even though file is uploaded

Hi guys,

I've seen this error intermittently.  My users upload upwards of 20+ attachments in the attachments tab.  They all show the file is uploaded.  Sometimes 1 or 2 cannot be downloaded and I get an error.  I've waited and tried to access the file again.  Still same issue.  It's not the first time I have seen this happen.
