How can I get this stl file model  converted to a model that will accept sketches and extrusions?

The scan quality of his file appears to be such terrible quality that I am wondering what you intend on doing with this ?

What is you end goal ?

The file is just to see what needs to be done get to a solid model.

There's an awesome video by @MichaelAubry about using Mesh files, MeshMixer, and Fusion 360 to get solid bodies.

Without looking the attached 21 MB file, my guess is that mesh is too dense for conversion. You can use the reduce tools in MeshMixer, to lower the triangle count. 

 

Another good tip is to use Autodesk Momento to convert your triangle Mesh into a Quad Mesh. This will make for a smoother conversion to T-Spline (Sculpt) Bodies > to BREP.

There is also the possibility to convert the STL file into a STEP file using InStep. It's really easy and not too expensive.
