Patch does not work as expected

I am doing my first design using Patch Workspace, it works as expected most of the time, but I have a situation in which the loop defined, there is no error message but the OK button is disabled. The only different I can think is that in this case the result patch is not a plane.  Earlier I finished the design succssesfully including non plan patch surface, but I restart and can not get it anymore. 

Attached are 2 screenshots - one that worked and the other does not.

Thanks ahead for any help!

I have a suspicion, but certainty will require the sharing of the design- File>Export>Save local>Attach the design to your response here.

Patch requires a closed loop; despite the appearance of closure, it seems Fusion doesn't recognize the end of one of those Line/Spline segments as connected, co-located Points.    Non-co-location/connection can stem from several sources, including operations, such as Projection, that are not a direct consequence of your workflow.    One guard against this condition is to take special care when joining segments that the target receives a Snap Recital Box around it before clicking.    For all those conditions when co-location has been botched by some process, there is an Ad-In tool for connecting  Points that are or are close to being co-located, called "ConnectTheDots"- find it here:

In use, (Window)Select the Points to Connect>Shift-S to bring up the Scripts and Add-Ins dialog>Select ConnectTheDots, and watch the magic happen.

This is much preferred to hiding the Sketch and attempting to Loft or Patch the existing Patch Edges, as difficult to perceive variations from the Sketch or design intent may be introduced, such as a fractional 4th edge.
