In design mode, I never see the option to set document settings

I want to tweak document settings in design mode. But I can't get the list of options to appear.

 Clicking on Document settings, or the gear icon next to it, doesn't do anything. Right clicking also doesn't. And I can't get a gear icon to appear to the RIGHT of document settings, which I've seen in some examples.

Hi, 

There is no list!

Only the units can be selected.
