In Solidworks, Solid Edge etc you can extrude a sketch up to a thin shell and the extrusion will only go as far as the outer skin, using "to" and selecting "next" in the extents field. In F360, it seems you can only make an extrusion with a final face that is parallel in its entirety to the original sketch. This often ends up extruding the sketch inside the original body where it isn't wanted.
For instance, if you were extrude a circular sketch onto a hollow sphere, the inside of the sphere would develop an unwanted flat circular or eliptical face. This happens with any relatively simple part that isn't very thick, where you want to extrude a boss or lugs onto it. Another example might be a deep drawn cup that you want to extrude some feet or fixings onto.
In the first screenshot you can see what results when I extrude a sketch toward the body, not far enough to merge fully. In the second one, you can see that it merges with the body but now it's also extruded into the other side of the body. "to" "next" would be rather helpful here!
Is there a solution to this? I can't even think of a simple work around to fiddle the result.

Yes, "to next" is a useful feature. It is in the backlog, but hasn't yet bubbled up to the top priority (but it's getting close). If you want to bump up its priority, I'd suggest putting an Idea Station request for it on the Fusion 360 Idea Station, and see what kind of response it gets from other customers.
