Any idea how I can have "90 deg" angles like this, and "vertical" constraints at angles?

I notice the arc at the bottom has an offset constraint, there have been some odd things like this when an offset is involved, try selecting the offset constraint and deleting it. You might need to redo the other constrains after deleting the offset, hard to know without the file.

If you'd be willing to share the model, we'd be happy to take a look at it.  It's hard to say offhand what the problem is, but it certainly seems like a bug.

I think I found the steps to reproduce the sketch:

The key step is adding dimension to one of the top line segment and later fix the point.

 

In fact we are having similiar problem without offset:

The two points at left bottom are coincident constrained but also failed.

Thanks for reply all.  I can almost always find work arounds- I just wanted to report issue to limit my (and others) workarounds in the future.  Glad you were able to replicate it, and apologies for my late response to your help.

 

I love to see this tool getting better and better.  I'm advocating it everywhere I go.
