Bidirectional Extrude to the same object. Impossible?

If I do two sided Extrude and try to select the same object (body or face) for both targets, I can't do that. Why?

This is a current limitation of Fusion. The extrude command cannot use the same target for two direction. In the case of round objects like you show, you are asking for the same face to be the target. If you were extruding to different faces it works.
