Running 2019 on a very good machine. Plenty of memory, processor, etc. Graphics card is Nvidia GeForce GTX 1060 with most current driver.
Performing any task such as drawing a line in 2D results in the cursor disappearing for several seconds before it returns ready for next task. Same for selecting objects, etc. Drawings are very, very small.
I've reset to default config, repaired the drawing (same problem on all drawings), etc., etc.
Any advice would be very appreciated.

- try that with new drawings based on AutoCAD's default templates
- if you login to your system with another user account
- disconnect your workstation from network (so it can't find Internet, network printers, network shares)?

The problem persists with all drawings and all templates.
To your point of logging in as another user, I just tried running ACAD as administrator using my usual User account, which seems to have corrected the problem - it performs at typical speed and is no longer "slow".
Any idea why?

Could be permission issues (e.g. access printers/plotters/folders/network shares faster with full permissions) or your AutoCAD profile points to locations not accessible without admin permissions.
Try to start AutoCAD as admin, start command _OPTIONS and on the tab "Profiles" export a profile.
Start AutoCAD in normal mode, run command _OPTIONS and import that ARG-file (created in the previous step).
Make sure this new imported profile is the default one, restart AutoCAD and try your performance behavior again.
