A couple of things --

First - it's generally inadvisable to use an image as a logo.  As you are discovering, problems crop up.  Far better in the long run to recreate/redraw the logo as native AutoCAD entities - line, hatches, etc.

 

Second, the attached image is attached using a relative path to your _template_ file.  As soon as you save to a different folder, that relative path is invalid.  The image will still display until you close the file, because it's in memory.  but when you open the newly saved drawing file, Acad cannot resolve the relative path.

 

As far as I know the only solution is to keep a copy of the image in every folder that you save the drawing(s) into.  (you could use a saved path all the way to the drive letter of the folder that has the original image is in, but that creates its own issues with portability etc)
