Main thing you should focus on is "Apply properties as viewport overrides." Is important to have this ON when you apply saved layer state on a layout.
It doeš not matter that much if you have this created in model or layout space (unless you did assign VP properties - right columns in layout layer properties dialog, which you did not).
BTW you cannot assing some specific Layer State to same specific Layout. This is just one-time thing, no other layout-NameState relationship.
Space column - is where this was originally saved (difference if you assigned VP properties).
Same as DWG - there are no VP properties, so it would be the same result no matter whether you apply this in model or layout.
