Multiple Annotations Appear and I do not know why
Several of my collogues have an issue where they have multiple annotation scales for a block and they do not know how they applied it. The additional annotative style affects every single dynamic block in the drawing, not just the ones they are using. It also applies to dimensions, etc.
The Automatically Add Annotation Scales button is un-checked. I am at a loss. This never happens to me but it happens to other people on my team.

It would be difficult for anyone to know how they were added if the automatically add button is off but if you just don't want to see the alternate scales when clicking on something you can set SELECTIONANNODISPLAY to 0

my answer is: I even don't know how it can happen, except the way you already said.
Sounds like a user mistake.
What's the name of the scale the added and what are the names which added then?
For example:
Just one they want and added: 1:100
and now the items have 1:100 and 1:50 and 1:200..

Model Space and the block were both set to 1" = 1'0" by default.
She selected the block and then clicked on the Annotative Scale button under the Properties tab and add 3/4" = 1'-0". That should only apply to that one block but it applied to each block in the file.

I'm not sure that's a thing, sorry. But I'm confused on how this is the same as your original post, I must have missed something in between.

Wow, that's sounds a bit different to what I understand firstly, but now I am more sure than before:
If really one BlockREFERENCE IN MODELspace selected, it is impossible.
Keep an eye on it (and the colleague).
Have you seen it yourself, reproduced it yourself in the workplace?

I have not experienced it myself, but I know one other colleague had the exact same thing happen to them. I think it happened to one other person as well. A mystery for sure.

That may be a reason for no solution so far: folks that have experienced what you described initially offering their tips, you're just translating from another user, changed the issue perhaps, and something somewhere is getting lost.
Videos of the problem in action or a stack of screenshots along with a command history (and a sample DWG file) will be the next steps for you if you still need to help ".. one other colleague had the exact same... I think it happened to one other person as well...".
