Your image shows it not when Inserting it, but when Editing its Attribute. Does it also appear that way in the INSERT dialog box? And when you type this at the command line:
(tblsearch "block" "test")
[replace test with the desired Block name], what is the case [or combination] of what it returns?
((0 . "BLOCK") (2 . "TEST") ....
And is it the same as the way it appears in the Insert or Attribute Editor dialog boxes? And is it the same regardless of what case combination you type in?

1. Single Click & insert (Become Lower Case) = Wrong
2. Click & Hold (As normal Upper Case) = correct

What happens when you use the CLASSICINSERT instead of the 'new' insert palette?
Do the cases also change?
I don't see this happening for me. What i do see in your screencast is that AutoCAD gives the block definition duplication error.
What happens when you perform the same thing in an AutoCAD template? Does it still change?
