It seems that the issue is that the original model view of the drawing was created on a .stb template
I followed other directions to copy/paste the information into a new drawing created on a .ctb template, which led to the results I described in my original post.
When I opened a new file using the basic acad.dwt, my .ctb plot style displays correctly in the viewport on a layout page. I am happy I now know the reason.
