OK, in 2019 version I can reproduce this issue. At first I opened your drawings in 2018 version and was not able to reproduce the problem in METRIC drawing.
Well, looks like there is a bug in 2019 version: when current coordinate system is not WCS (World coordinate system), but some other UCS (like you have in METRIC drawing - look at view cube - you will see Unnamed instead of WCS) dimensions start to behave as you see with ortho mode ON.
If you will switch from UCS to WCS in METRIC drawing, dimensions will behave as they should.
To switch to WCS, use UCS command and hit Return when AutoCAD will ask for origin point:
Command: UCS
Current ucs name: *NO NAME*
Specify origin of UCS or [Face/NAmed/OBject/Previous/View/World/X/Y/Z/ZAxis] <World>: <-Hit Return here
